The forms below are for use by scheme employers.

Starters, leavers and changes of contract

 

PSR2014 - Pensions Service Record form to be completed by the employer when an employee joins the LGPS  (Word 43KB)

 

TERM2014 - (Word 49KB) to be completed by the employer when a member leaves the LGPS (for employees members who have paid into to the main section of the LGPS only in the financial year in which they leave).  Please read our guidance notes for more information on how to complete this form.

 

TERM5050 - (Word 61KB) to be completed by the employer when a member leaves the LGPS (for employees who have contributed to the 50/50 section of the scheme for part or all of the financial year in which they leave).  Please read our guidance notes for more information on how to complete this form.

Following changes to the LGPS regulations from 1 April 2026 relating to periods of absence, these forms and the guide are currently under review and will be updated soon.
