The documents below are sample ill health certificates to be used by employers participating in the LGPS.

2014IH1 - Certificate for a current employee (Word 49KB)

  

2014IH2 - Certificate for a deferred beneficiary who ceased membership as an employee on or after 1 April 2014 (Word 40KB)

2014IH8 - Certificate for a 3rd tier ill health retirement review for a current pensioner where the cessation of employment occurred after 31 March 2014 and the review is taking place 18 months after the date of cessation of employment (Word 43KB)

 

2014IH9 - Certificate for a 3rd tier ill health retirement review for a suspended pensioner where the cessation of employment occurred after 31 March 2014 and the review is taking place at request of the member while the 3rd tier pension is in payment or within 3 years after payment of the 3rd tier pension has been discontinued (Word 46KB)

 

2014IH10 - Certificate for a 3rd tier ill health retirement review for a suspended pensioner where the cessation of employment occurred after 31 March 2014 and the review is taking place 3 or more years after the date of cessation of 3rd tier pension (Word 40KB)

 

2014IH12 - Certificate for a 3rd tier ill health retirement review for a suspended 3rd tier pensioner where the cessation of employment occurred before 1 April 2014 and the review is taking place within 3 years of the date of cessation of the 3rd tier pension (Word 47KB)
