The forms listed below can be downloaded and used by employing authorities to notify Bedfordshire Pension Fund of changes regarding Local Government Pension Scheme members.
PSR2014 - Pensions Service Record form to be completed by the employer when an employee joins the LGPS (Word 43KB)
TERM2014 - (Word 49KB) Notification of a member leaving the scheme (for members who have contributed to the main section of the scheme only in the financial year in which they leave). Please read our guidance notes for more information on how to complete this form.
TERM5050 - (Word 61KB) Notification of a member leaving the scheme (for members who have contributed to the 50/50 section of the scheme for part or all of the financial year in which they leave). Please read our guidance notes for more information on how to complete this form.
