The forms listed below can be downloaded and used by employing authorities to notify Pensions Administration of changes regarding LGPS members.

 

PSR2014 - Pensions Service Record form (For new members joining 01/04/14 or later) (Word 43KB)

 

TERM2014 - (Word 49KB) Notification of a member leaving the scheme (for members who have contributed to the main section of the scheme only in the financial year in which they leave).  Please read our guidance notes for more information on how to complete this form.

TERM5050 - (Word 61KB) Notification of a member leaving the scheme (for members who have contributed to the 50/50 section of the scheme for part or all of the financial year in which they leave).  Please read our guidance notes for more information on how to complete this form.
